    Headline and standfirst revised. The original headline ("Bulk-Billing Is Collapsing — But Not Where You'd Expect") framed the finding as counter-intuitive. It is not: bulk-billing incentives are means-tested, so a higher rate in poorer areas is the designed outcome, and presenting it as unexpected overstated the result. The analysis, figures and charts are unchanged; the framing now leads on the size of the gap and the growing uselessness of the national average.
